Wake Family Medicine is an approved site to conduct Immigration Medical Examinations. Medical exams are required for an adjustment of status. Our office is proud to offer complete, thorough, and concise medical exams for you and your entire family. Our physician is a designated Civil Surgeon for the U.S. Citizenship and Immigration Services (USCIS) through the Department of Homeland Security.

USCIS exams consists of a minimum of 2 visits, generally about few days apart. The first visit consists of a review of your vaccination documentation, a detailed history and a physical exam and lab work. The second appointment is to review your lab results and complete the application. To prepare for your appointment, visit the USCIS website. A download of the i-693 application is available on their site. Bring your completed i-693 and your legal identification to your appointment.

DO NOT sign your i-693 form. Your signature must be completed in front of the physician. The physician will complete the Civil Surgeon-required portions of your application. The original documents will be placed in a sealed envelope for USCIS and a copy of the documents will be given to you.

Medical services needed to pass the exam:

  • Physical Examination and evaluation of other Physical or Mental abnormality, disease or disability
  • Documenting and completion of all the necessary paperwork
  • QuantiFERON and interpretation
  • RPR (Syphilis test)
  • Review Vaccination record
  • Administer any needed vaccinations

Patients are advised to bring the following to their appointment:

  • Government issued photo ID such as a driver’s license or passport
  • A copy of any prior immunization/vaccinations records
  • Prior diagnostic test if any
  • Any mental health records
  • A completed but not-signed version of the most recent Immigration Physical Form i-693

Children (2 years of age to 14 years)

  • Tuberculosis (QuintiFERON)

Age 15 and above

  • Tuberculosis (QuintiFERON)
  • Syphilis
  • Gonorrhea

Required Vaccines:

Children (2 years to 14 years)

  • ALL school-required childhood immunizations

Age 15 and above

  • MMR
  • Influenza (seasonal requirement)
  • Hepatitis A
  • Hepatitis B
  • Varicella
  • Meningococcal

After The Exam:

Once the immigration physical examination is completed, our office will certify the completed Form i-693 with the test results of the immigration exam and vaccination history. Please present this sealed envelope to Department of Homeland Security and U.S. Citizen and Immigration Services with your immigration application. You will also be provided with a copy of these forms.
